Taylored Instruction – Agreement, Waiver, and Release of Liability

I, the Participant or the Parent/Guardian of the participant, having thoroughly reviewed the descriptions of the Program(s) offered by Taylored Instruction LLC, hereby agree, in consideration of being allowed to participate in these Programs, to waive, release, and discharge Taylored Instruction LLC, its instructors, agents, and employees from any claims for personal injury, death, or property damage that may arise from participation in the Programs. This waiver and release cover all liabilities arising from negligence or carelessness of Taylored Instruction LLC or otherwise. I acknowledge that participation in the Programs involves inherent risks, including the risk of physical injury or death, and I assume all related risks despite these dangers. I understand that participation in the Programs does not guarantee complete safety or being water-safe.

Furthermore, I agree to indemnify and hold Taylored Instruction LLC and its representatives harmless from any loss, liability, damage, or cost they may incur due to participation in the Programs, whether caused by negligence or otherwise. As the Participant or the parent/guardian of the Participant, I consent to participation in the Programs and agree to the terms of this Agreement, Waiver, and Release on their behalf, affirming that the Participant is physically capable of participating in the Program(s). I have read this Agreement, Waiver, and Release, understand its terms, and sign it voluntarily. I recognize that this document constitutes a release of liability and a contract between Taylored Instruction LLC and myself, and I sign it of my own free will.
